Safety protocols have been established to minimize health risks to students and school staff.   Daily health screening of students by parents will be necessary.  Before entering school buildings, families will be asked to submit an Electronic Health Screen Form prior to their child’s arrival to school each day.  Health screening questions will ask if a student has COVID-19 symptoms, been in close contact with anyone confirmed with COVID-19, tested positive for COVID-19 within the last 10 days, and been given advice from a medical professional to self-monitor, self-isolate, or self-quarantine.

    Frequently Asked Questions:

    What is the Daily Health Screening?

     • A set of questions to be answered daily regarding symptoms, exposure, or positive test for COVID-19

     Why are we asking for it?

     • The Department of Health requires all students, staff, vendors, parent/guardians or visitors to be screened. Individuals with symptoms, close contact exposure, or who have tested positive for COVID-19 may not enter a school or other district facility. This also includes school buses and other district provided transportation.

     What do we do with the information?

     • The data is collected and reviewed each day by staff in our schools

    How do I access it?

     • District website

     • A QR code will also be posted on the main entrance door to the school

     • If you are having difficulty with the online form you may call your child’s school and answer the screening questions over the phone

    What questions am I going to answer?

     • Your child’s name, grade, school and teacher

     • Do you have any of the following symptoms that are not caused by another condition? Fever (100.4F) or chills, cough, shortness of breath or difficulty breathing, unusual fatigue, muscle or body aches, headache, recent loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea?

     • Have you been in close contact (within 6ft for 15 minutes or longer) with anyone with confirmed COVID-19?

     • Have you had a positive COVID-19 test for active virus in the past 10 days?

     • Within the past 14 days, has a public health or medical professional told you to self-monitor, self-isolate, or self-quarantine because of concerns about COVID-19?

    When do I complete the Electronic Health Screen Form?

     • Every morning BEFORE your child enters a school district building or boards the school bus.

    Do I need to complete it every day?

     • Yes, the Department of Health and the Puyallup School District requires the daily screening to be completed each day before coming to school

    What if I have more than one student attending in-person learning?

     • A daily Electronic Health Screen Form will need to be completed for each student, each day.

    Can my child come to school if I answer “yes” to any of the questions?

     • No. You will need to contact your student’s nurse/health room for information on timelines for returning to school.
